Adults over age 55 represent the fastest-growing demographic of patients with epilepsy, yet diagnosing the underlying cause in this population remains a complex clinical challenge. While acute ischemic stroke is a well-established driver of late-onset seizures [1, 2], the International League Against Epilepsy emphasizes that etiology must be reassessed at every diagnostic stage due to its profound impact on long-term management [3, 4]. Emerging evidence suggests that metabolic interventions, such as those involving newer glucose-lowering agents, may modify the risk of developing these neurological events [5]. However, as the geriatric population expands, clinicians must increasingly differentiate between vascular insults and the subtle onset of age-related encephalopathies [6]. A recent multicenter study provides an updated characterization of these seizure triggers across diverse patient populations, offering insights that can directly inform risk stratification and diagnostic workups in daily practice.
