Nebulized Heparin Linked to Reduced Mortality, More Ventilation-Free Days in Respiratory Failure

Acute respiratory failure remains a primary driver of morbidity and mortality in critical care, with a complex pathophysiology that extends beyond simple gas exchange impairment [1, 2]. A key feature in many etiologies is a localized coagulopathy within the lungs, where excessive fibrin deposition contributes to alveolar collapse, impaired oxygenation, and the progression of lung injury [3, 4]. This has led to investigations of therapies that can act directly at the site of injury, such as nebulized unfractionated heparin, which leverages its anticoagulant and anti-inflammatory properties within the pulmonary space [5, 6]. While individual trials have explored this strategy in settings like COVID-19 and inhalation injury, their small sample sizes have produced inconsistent findings [7, 8, 9, 10, 11]. A new meta-analysis of 16 randomized trials involving 1,620 patients now provides a more definitive picture, finding that nebulized heparin was associated with reduced all-cause mortality (risk ratio, 0.79; 95% CI, 0.66-0.95) and an increase of 4.85 ventilation-free days (95% CI, 1.47-8.24), without a significant increase in major bleeding [12].
