Selective HDAC6 Inhibition Improves Motor Function in Preclinical ALS Models

The clinical management of amyotrophic lateral sclerosis and frontotemporal dementia remains a profound challenge due to the complex failure of cellular quality control systems. Central to these pathologies is the breakdown of proteostasis, the process of maintaining proper protein folding and clearance, which leads to the accumulation of misfolded aggregates that trigger neurotoxic cascades [1, 2]. Chronic neuroinflammation, driven by the activation of microglia and astrocytes, further exacerbates this environment and accelerates progressive motor and cognitive decline [3, 4]. While previous attempts to utilize histone deacetylase inhibitors (drugs targeting enzymes that regulate protein stability and gene expression) showed early efficacy in preclinical models, clinical translation was halted by poor drug selectivity and inadequate penetration of the blood-brain barrier [5, 6]. A recent study [7] investigates whether a highly targeted pharmacological approach can successfully address these fundamental transport and clearance deficits, potentially offering a new avenue for disease modification.
